PCT Tick List for Trainees rotating into GP Posts

    • Contact the relevant Agency/PCT to ask for the paperwork to join the Medical Performers List for the area in which you will be working. Please be aware this process will take approximately 10 weeks to complete.

    • Complete PAY1 form – check the training dates for that placement are correct.

    • Attach a copy of your passport (to verify date of birth), current CV and a copy of a payslip from your current employment, current Certificate of Annual Registration of the GMC and Evidence of Membership of a recognised professional medical defence organisation.

    • Pass the PAY1 form and supporting documentation to your trainer. They need to sign and date this document.

    • Ensure that the completed PAY1 form and supporting documentation is sent to the Deanery for signing. It is a good idea to keep a copy of the form in case it goes astray in the post.

    • Ensure that the PAY1 form is completed promptly to ensure you will be paid on time. Failure to do so may mean that your salary is delayed.
